About
Failsafe Network, Inc. was founded in 1985. We are excited to celebrate our 40th anniversary, helping people learn from things that go wrong. We have developed a unique process called Latent Cause Analysis (LCA). This process is highly effective at helping people understand both ‘how’ and ‘why’ an event occurred, and it is also excellent at developing leadership qualities.
Our understanding of things that go wrong has changed considerably since 1985. Whereas we initially helped people understand and address the PHYSICS of their problems, that is, "how things break," we are now helping people to address the underlying human aspects of just about everything that goes wrong in our lives.
In other words, we have gathered a vast range of experience and accumulated knowledge about Latent Cause Analysis™, Root Cause Analysis, Human Performance, and Leadership.
We help businesses improve by understanding Latency, a significant cause of all problems. We offer a life-changing 4-day experience that equips people to identify and address Latency. In the process, people leave with a completely different perspective on what's causing problems where they work. Our company was built on the foundation of Latent Cause Analysis™, which is significantly different from Root Cause Analysis. Latent Cause Analysis™ (LCA) leads to profound learning.
We provide customized classroom training, online resources, and consulting services for an entire organization, from executives to managers to line-level employees.

Leadership
C. Robert (Bob) Nelms is an innovator in causal analysis and the founder of Failsafe Network, Inc. He is the author of two books, “What You Can Learn from Things that Go Wrong” and “The Dynamics of Inculcating a Root Cause Mentality.” Nelms is an Aerospace Engineer who graduated from Texas A&M in 1969.
Rob Statham is the President of Failsafe Network, Inc. He has over 30 years of experience in the oil and gas industry. Rob is an expert trainer and a seasoned principal investigator passionate about helping people learn from things that go wrong. He has a unique ability to connect with all levels of an organization and specializes in creating robust learning teams for enhancing human and organizational performance. Rob firmly believes that culture change starts with understanding Latency.
Failsafe is passionate about helping people learn from things that go wrong and to help people “see themselves” as part of their problems.
