 Operation
Failsafe is the UMBRELLA strategy under which all of
Failsafe's training and consulting services dwell. Operation Failsafe
recognizes the need to provide a plan to help people learn from
things that go wrong (root cause
analysis) in your workforce, as well as providing a practical means
of executing the plan. It was developed by Failsafe to help you
and everyone in your organization identify and act on the real causes of things that go wrong.
Operation Failsafe recognizes two situations in which many
people find themselves.
Sometimes, individual employees often find
themselves working for employers that are in destructively reactive,
firefighting modes. Instead of trying to learn from things that go
wrong, these employers seem to be hopelessly stuck in the rut of mindless

Other employees often find themselves
working for employers that have heard of "Root Cause Analysis," but want to
learn more. Some of these employers (high hazard industries, including
health care) might even be required to start performing Root Cause
