Failsafe's
top-rated seminar
|
A VERY interactive, engaging session that will
change the way you think about "failure" and it's causes.
Organizations that want to change their cultures (by
changing the way their people think) routinely host this
seminar at their sites, year after year.
An investigative method is taught. A
real incident is investigated. Most importantly,
however, attendees will have a new attitude about things
that go wrong, including a healthy realization of their own
role.
Designed for EVERYONE, including
operators, maintenance craftspeople, technical personnel,
health and safety, quality, reliability, management, union,
secretarial -- EVERYONE.

Detailed Description...........
Several times per year, one of Failsafe client's opens-up their
on-site Latent Cause Experience seminar to outsiders. This helps
the client think "outside the box" by bringing-in outside
perspectives. It gives the outsider a first-hand look at the
way one organization responds to "The Latent Cause
Experience." Whether you are testing The Latent Cause Experience
for your own use, or are a newer member of an already-trained
organization, this will probably be one of your most
memorable training experiences.
Don't be fooled. This is not a class
designed only for "Principal Investigators," nor any one
specific kind of people. This class has evolved into a
4-day series of "messages," profound and simple messages.
Perhaps the most important message is really a question:
What is it about the way I am that contributes
to our problems?
Imagine an organization whose employees are
willing and able to ask, then act on this question.
Really. Imagine.
Includes:
- What is Latent Cause Analysis?
- The Lessons of the Challenger Explosion
- How to Inculcate a Latent Cause Mentality at Your
Site
- The Maxi, Midi, and Mini-LCA
- How to Set Triggers for Performing Latent Cause
Analyses
- The ROOTS Investigative Method
- Evidence, the Lifeblood of the Latent Cause
Analysis
- The Vector Approach to Evidence Gathering
- How to "Let the Evidence Guide You"
- How to Identify the Stakeholders, Based on the
Evidence
- Involving the Stakeholders in DISCOVERY
- When and How to Construct the the WHY Tree
- How to Identify the LATENT CAUSES of the Incident
- How to Translate your Findings
- How to Sustain the Latent Cause Analysis Effort
Throughout the 4-day class, you will be applying your
learning's to a real incident by reviewing real evidence.
In the process, many dialogues occur amongst class participants
and the instructor. The highly interactive nature of the class involves everyone for
an enjoyable, memorable, and immediately usable experience.
"I wish everyone in our organization, top to bottom, would go
through this experience"
