Latent Cause Analysis (Failsafe's Brand of Root Cause
Analysis).
Contrary to what you might
think, you are probably NOT doing Latent Cause Analysis
right now. True Latent Cause Analysis requires the
inquirer to look at THEMSELVES, asking "what
is it about the way I am that contributed to this incident."
Latent Cause Analysis should
be something that EVERYONE does, habitually.
Eventually, it will be something that people do "in their
minds."
Imagine what might happen if a whole
organization started looking at themselves instead of
blaming their problems on other things and people.

This 3 1/2 hour live and interactive session
will take the attendee through the Mini-LCA process:
- a process designed for EVERYONE in an
organization
- to be used on ANY problem (preferably a
SMALL problem)
- to quickly gather a LOT of information
about "latency"
- so that the latencies can be addressed
PRIOR to causing a big problem.
Attendees will be taught:
- Why Latent Cause Analysis must be for
everyone.
- The difference between a Maxi, Midi,
and Mini-LCA
- The difference between Physical, Human,
and Latent Causes
- The 3 forms of Evidence (3Ps) to always
consider
- How to do a Mini-LCA
Attendees will be taken through the
process in a step-by-step manner, and
will perform an actual Mini-LCA on a real
chemical plant. Plenty of opportunity will be available to
interact with one another, sharing one another's reactions
to the training.
