Failsafe Network has tapped into an international group of
practicing rooticians from diverse industries and businesses to help
determine what is possible in the root cause analysis world. Using
information obtained from this world-wide source, we have developed two
types of assessments.
Our first type of assessment is very generic, and
can be applied no matter what type of investigative method is being used
within an organization.
Our 2nd type of assessment is more specific, and
was developed to help organizations that have committed to Failsafe's
investigative methods.
Both of these assessments are offered free of charge
to anyone desiring to do a SELF-ASSESSMENT. We would also be
delighted to facilitate a more formal assessment if you desire.

Enabling the Mini-RCA/LCA Process

It is a gross mistake to merely wait for a catastrophic
event to occur, and then to investigate it. We have learned, the
hard way, that catastrophic events are caused by the same factors that allow
small problems to fester. It's the small problems that cause big
problems. Knowing this, Failsafe Network has
developed a specific strategy for helping organizations to learn from their
small problems. The backbone of this strategy is abundant use of
the Mini-RCA/LCA. |

Part of Failsafe's suggest approach to learning from things
that go wrong is to track, trend, and then act on organization latent
causes. However, instead of using pick-lists (or drop-down
lists) to choose organizational latent cause categories, Failsafe has
developed a more flexible, open-ended approach that produces more meaningful
results. |

Since 1985, Failsafe has been insisting on the need for
organizations to have dedicated groups of people focused on learning
from things that go wrong (root cause analysis). Without these kinds
of groups, most people get sucked onto the "Merry-Go-Round" without even
knowing it -- until disaster strikes in one form or another.
Failsafe calls these dedicated groups of people a Root
Cause Analysis Mother-Source, because they must nurture the
endeavor of "learning from things that go wrong."
Unfortunately, however, most organizations do not have
this resource. They think they are already stretched too thin.
Failsafe's has relationships with several Root Cause
Analysis Consultants (independent consultants), who are Licensed
by Failsafe to fill this essential gap until you can justify a group of your
own.
