In 2001, Failsafe
recognized the need to formally recognize people whose peers thought
were potential Root Cause Analysis champions. Root Cause Analysis,
perhaps more than any other
endeavor, requires PASSION and COMPASSION.
Even more, a successful RCA effort requires an AGGREGATION of CHAMPIONS.
Lone champions tend to get burned-out very quickly in this field.
Therefore, this Hall of Fame was created for three main purposes:

to publicly recognize
the people who were voted CHAMPIONS by their peers.

to grow a group of
people who will hopefully stay in touch with one another -- to build
upon each other's enthusiasm and knowledge.

to challenge those who
were not voted into the Hall of Fame to
do something about their passion for learning from things that go wrong.
Prove the voter's wrong! Show that YOU are a CHAMPION.

The on-site Root Cause Experience is an
interactive, team-based
encounter. Each day, people are placed in different teams so
that each can get to know one another's perspectives.

On the last day of the 4-day
Experience, a special team-based wrap-up session is performed.
Team photos are taken during this wrap-up.
Try to find yourself, your friends, and
some site-specific photos in some cases.
