Operation Failsafe's Individual Path

If you are reading this page, your are either personally interested in this subject, or you have been asked (or told) to "get trained" in this subject by someone else.  If you have been told to be trained in this subject, our hope and mission is to stimulate your interest -- hopefully to tap into your innate need to know why things go wrong.  If we can activate this latent interest within you and focus it on "small problems," you'll be a positive and powerful influence wherever you are.

On the other hand, you might already have a keen interest in this subject, but find yourself in an organization that does NOT share your interest.  If so, please recognize that you are in the majority -- it is rare to find an organization whose management consistently demands a "root cause understanding" of things that go wrong.  Sure, many organizations reactively investigate their large problems.  But few organizations are willing to invest the time and energy into involving everyone in learning from small problems in order to prevent the big ones.

It is more likely to find isolated individuals within an organization, perhaps like yourself, who are tired of ignoring the spinning, whirling, nonsensical "the Merry-Go-Round" -- not whiners and complainers, but DOERS -- people with positive attitudes who desire to make the world a better place, champions in their own right.

Don't be part of the problem.  Be part of the SOLUTION!  DO SOMETHING about the Merry-Go-Round!

Are you this kind of person, keenly interested in this field of "Root Cause Analysis?"  Is this an endeavor you consider worthy, something you'd like to pursue?  Or have you been told to pursue this kind of training?  Whichever person you are, Failsafe would like to help -- not because we have all the answers, but because we have the same interests and we see the same value. 

We'll do our best to help you change, so that others can follow your lead. 

That's the key -- LEAD BY EXAMPLE.  You cannot expect others to do something you're not willing to do, or think in a way you're not willing to think.  On the other hand, miracles occur when individual people like you take aYou have to know what you're looking for in order to find it! stand and intentionally try to see themselves as part of the problem instead of merely blaming someone (or something) else.  We're not talking here about a masochistic, nonsensical, self flagellation.  We are talking about the sincere desire to set an example by looking beyond the surface of a problem instead of merely reacting.  People will NOTICE if you consistently behave in this manner.

  • Join our mailing list.

  • Listen to an Introductory, free, 15 minute Web entitled "What is Root Cause Analysis?"

  • Encourage others to listen to it.  Talk about it with others.

  • Attend The Latent Cause Experience.  This class is offered many times per year at various locations around the world. 

  • Attend Latent Cause Analysis for Everyone.  This class is offered every other month, on the web.

  • Perform a Mini-LCA on any real home problem.  Send it to Failsafe for constructive comments.
