Welcome to the third issue of Failsafe's new NETLETTER.

This month, I'm taking a departure from my policy of elaborating on a particular Root Cause Analysis issue to introduce major new initiative. Actually, the initiative DOES address the issue of INCULCATION, i.e., ingraining a root cause mentality throughout an organization. Most of you are struggling with implementing your root cause efforts.

People like myself (consultants and trainers) get frustrated when they see positive results in the training room that do not migrate into the workplace. It's obvious that a once-a-year on-site injection of enthusiasm, energy, and training is NOT ENOUGH to sustain a root cause analysis effort. Necessary, but not sufficient. Sites are in need of an easy, inexpensive, and continuous way to immerse their people in root cause analysis thinking.

I have high hopes that you will see the value in what I'm introducing in this Netletter.

Most of you know about The Root Cause Experience, the 4-day, on-site seminar designed to jump-start a new or lackluster Root Cause effort. This has become an increasingly popular seminar; I've conducted it 50 times in the last 2 years, from Washington to Pennsylvania, Canada to Venezuela, England to Thailand.

Each time I conduct this seminar, I feel as if I'm actually leading a Root Cause Analysis. Because we investigate an actual incident, we encounter just about all the things a real organization confronts in its quest to understand why something went wrong. Because of this continual exposure to investigative impediments, I have become increasingly aware of the need to provide reinforcement and followup training; specific, brief, and "just-in-time" information FOR THE MASSES, AFTER a significant cross-section of your people have been energized by The Root Cause Experience.

Failsafe University (FSU) is being created to make some of the segments of The Root Cause Experience available "to the masses" on an as-needed basis. In addition, FSU will provide supplemental training in areas not covered in The Root Cause Experience. Finally, FSU will allow people who are unfamiliar with Root Cause Analysis to get some initial, inexpensive exposure.
