not attempt to identify the Latent Causes of an incident without using the
Situation-Filter-Outcome (SFO) Model. The SFO model has been modified
many times since being developed in the early 1990's. All of
Failsafe's failures in trying to identify Latent Causes have been addressed
by using this model. It is strongly suggested that you download the
PowerPoint slides, then show them to the stakeholders during the Latent
Cause phase of the stakeholder meetings. It will keep everyone on
track, and reveal what needs to be revealed to help everyone understand the
Latencies.
