Failsafe
Network, Inc. moderates 3 forums related to
Root Cause Analysis. All forums use YahooGroups, an opt-in,
opt-out, free web-based forum. CLICK ON THE NAME OF THE
FORUM TO JOIN.
THE FAILSAFE FORUM: The
Failsafe Forum is "communications central" for people interested in
applying the root cause analysis (latent cause analysis™) methods taught by Failsafe Network, Inc. Any and all
dialogue between Failsafe's users should occur on this forum for the benefit
of all.
Join the forum if you would like to learn more about Failsafe's
root cause analysis and latent cause analysis methods, discuss successes and failures in applying the
methods, or if you are in need of help. In this respect, all of
Failsafe's Licensed Root Cause Analysis Consultants are members of this forum and read all the
messages.
Competing consultants are NOT welcome on this forum. People
trained in other root cause analysis techniques are welcome, but only if their intent is to
understand more about Failsafe's root cause analysis and latent cause
analysis approaches. Any attempt to promote other
methods, divert people to other (competing) forums, or to confuse people in
any way will be cause for immediate rejection from this forum.
THE OPEN MOTHER-SOURCE: The Open Mother Source is "by
invitation only." It is an inter-company, inter-industry group of
champions who have agreed to function just as a site Mother-Source
might function. As such, we meet routinely to plan for the future,
address current problems, and further advance our intent to learn from
things that go wrong. If you would like more information about the
OMS, or if you would like to belong to such a group,
please
write.
ROOT CAUSE CONFERENCE: For those interested in other Root Cause Analysis methods and forums,
please consider the "Root Cause Conference," a forum initiated and moderated by C. Robert
Nelms for those on the leading edge of Root Cause Analysis. Here you
will find over 1,000 practitioners from all over the world, including most
of the recognized leaders in the field of Root Cause Analysis. BEWARE,
however! You will hear about a variety of approaches and methods, many
of them diametrically opposed to one another. This forum is for the
advanced practitioner.
